Why do staff in special needs schools need the hepatitis B vaccine?

Hepatitis B is a blood-borne virus that can be transmitted by needlestick injury, bites that break the skin, scratches, exposure to bodily fluids, and intimate-care contact. The UK Green Book Chapter 18 (updated February 2026) specifically identifies staff working in day-care settings and special schools for those with severe learning disabilities as an occupational group for whom hepatitis B vaccination should be considered, on the basis of a local risk assessment.

The risks in special needs school environments are well-documented. A 2022 study of 1,398 special school staff (Lang et al., Scientific Reports) found that between 66% and 93% of staff had been scratched, bitten or spat at by pupils. Daily physical contact was reported by up to 84% of all employees. Despite the high exposure, only 64% of staff had completed a hepatitis B course — and the largest single predictor of remaining unvaccinated was lack of structured access to vaccine counselling. The barrier is access, not hesitancy.

Activities that carry hepatitis B risk in special needs schools include:

  • Intimate care, including toileting, continence support and catheterisation
  • First aid for cuts, scrapes and bleeding wounds — which 94–98% of school staff perform at some point
  • Managing behaviour that includes biting, scratching or spitting — common in learners with profound and multiple learning difficulties

Do employers have to pay for the hepatitis B vaccine?

Yes. Under the Control of Substances Hazardous to Health Regulations 2002 (COSHH), every employer must assess the risk of exposure to biological agents — including hepatitis B virus — and, where the risk is significant, provide vaccination at no cost to the employee. The British Medical Association’s published guidance is clear: occupational hepatitis B is the employer’s financial responsibility, and NHS GP practices cannot charge a registered patient for occupational hepatitis B vaccination.

For special needs schools, academies and multi-academy trusts, this means the school or trust commissions and pays for the programme on behalf of its staff. Staff bear no cost. Failing to provide vaccination where a meaningful risk has been identified exposes the school to potential HSE enforcement action and to civil liability if a member of staff later contracts hepatitis B at work.

The same duty applies to teaching assistants, learning support assistants, behavioural support staff, school nurses, physiotherapists, occupational therapists, midday supervisors, transport escorts, cleaning staff who handle clinical waste, and anyone else with regular intimate-care or first-aid responsibilities. The duty applies whether the school is maintained, an academy, an independent provider or a specialist independent school.

Hepatitis B vaccination schedules

We offer all three Green Book-compliant hepatitis B vaccination schedules. The right choice depends on how quickly protection is needed and on how easily staff can be released from teaching duties. The accelerated schedule and very rapid schedule are particularly useful for new starters who need protection before they begin regular intimate-care duties.

Post-vaccination antibody testing (anti-HBs titre)

Around 4 to 8 weeks after the third dose, we offer a confirmatory anti-HBs antibody blood test, with results recorded against each staff member. A titre of ≥10 mIU/mL confirms protective response. Non-responders — around 5 to 15% of healthy adults, more common in smokers, those with a BMI above 30, and people aged over 40 — are offered a repeat course or further investigation in line with Green Book guidance. Without an antibody titre check, the school cannot demonstrate that the vaccination programme has actually conferred protection, which is the whole point of running it.

Hepatitis B booster doses

Current UK guidance does not routinely recommend booster doses for healthy adults who have had a documented anti-HBs response above 10 mIU/mL. We document this clearly on each staff record so that schools have an audit-ready position when the question is raised by occupational health auditors or inspectors.

Records, certificates and reporting

Every staff member receives a personal vaccination record card and a digital certificate. The school receives a coded staff register showing course completion and antibody confirmation status, formatted for occupational health audit. Records are retained securely in line with GDPR, CQC and HSE requirements. Reports can be issued directly to the school’s occupational health provider on request.
