The MED3 Statement of Fitness for Work is a Department for Work and Pensions form available only through NHS-funded care. As a private clinic, we are not able to issue one.

What we provide is a private medical certificate. Some employers, schools and universities accept a private certificate in place of a fit note, but they are not obliged to, and acceptance is entirely at their discretion. A private medical certificate cannot be used to support a claim for Statutory Sick Pay where your employer specifically requires a MED3, and it cannot be used to support a claim for health-related benefits such as Universal Credit or ESA.

MED3 Fit Notes and Private Medical Certificates: What's the Difference?

The MED3 fit note (formally, the Statement of Fitness for Work) is a Department for Work and Pensions form. It is issued free of charge through NHS-funded care by a doctor, nurse, occupational therapist, pharmacist or physiotherapist responsible for your care. It is the document DWP requires to support a claim for health-related benefits, and it is the document most employers’ sickness policies name by default. Private clinics do not have access to the MED3 form and cannot issue it.

A private medical certificate is a letter from a GMC-registered doctor confirming, after a clinical assessment, that you have a health condition affecting your ability to work or study, and for what period. It has no statutory standing. Government guidance confirms that a private certificate may be accepted by an employer as medical evidence in the same way as a fit note, but only where that employer agrees to accept it.

In practice this means:

  • You can self-certify for the first seven calendar days of sickness absence. No certificate of any kind is needed during that period.
  • After seven days, most employers ask for a fit note. Many accept a private certificate instead, particularly where the alternative is a delay in being seen. Some will not.
  • If you need evidence to support a benefits claim, only a MED3 from NHS-funded care will do.
  • If your employer’s policy specifically names a MED3, check with your HR department before booking a private appointment.
