Why this area is treated differently

The skin of the lower eyelid is substantially thinner than elsewhere on the face, has little underlying support and moves constantly. Loss of hydration and elasticity therefore shows here first, and the area tolerates aggressive intervention poorly. Polynucleotides are placed superficially in small volumes, and technique matters more here than on the cheek or neck.

What it can and cannot address

Treatment may help where the concern is fine lines, crepey or thin-looking skin, dehydration or reduced elasticity. It will not correct hollowing of the tear trough, will not remove eye bags caused by fat prolapse, and will not treat pigmentation.

Dark circles are the most common reason patients ask about this treatment, and they are not a single condition. The appearance may come from pigmentation, from vessels showing through thinned skin, from shadow cast by a hollow, or from a combination. Polynucleotides help with the component caused by poor skin quality. Assessment should establish which predominates before treatment is recommended; whether it is pigmentation or volume loss, we will say so.

Polynucleotides or tear trough filler

Tear trough filler replaces volume in a hollow and is a technically demanding treatment with a recognised rate of prolonged swelling and irregularity. Polynucleotides add no volume, so a hollow remains a hollow. Where both skin quality and volume loss are present, treating skin quality first is the more conservative sequence and can reduce the filler subsequently needed.

Treatment and aftercare

The area is cleansed, and the product is placed through a series of fine injections, taking fifteen to twenty minutes. Small bumps are visible immediately and generally settle within hours. Mild swelling for a day or two is common and more noticeable here than elsewhere; bruising is possible. Avoid make-up on the area, strenuous exercise and heat for the rest of the day.

Treatment is not carried out during pregnancy or breastfeeding, where there is active infection or inflammation around the eye, or where there is known fish allergy. Blepharitis or active eczema should be settled first.
